Questionnaire

Responses

The answers you supply in the questionnaire will be used to supply you with specific tailored advice about your post-reproductive health. You are given the option before starting the questionnaire on how you would like your information treated.

Supplying an email address

If you supply an email address to receive your MmM advice via email as a PDF atachment you are given three options as to how you would like to be kept updated or contacted in the future. These options are as follows:

  • for post-reproductive health research
  • for website feedback
  • for notification of significant updates to the ‘Manage my Menopause’ portal

You do not have to select any of these options but if you do all we will do is use your email address for the specific purpose selected.

Save and Continue Later

If you utilise the “Save and Continue Later” feature of the questionnaire, your responses up to the point you clicked the “Save and Continue Later” button will be stored in the database for 30 days to allow you time to complete the form. After 30 days the responses you have provided so far will be deleted from the system. You will also need to provide an email address to use the “Save and Continue Later” feature so that we can email you the link to continue the questionnaire.

Cookies
